The protocol

PerioblastTM: Periodontal Bio Laser Assisted Therapy is the result of years of studies applying scientific research and cutting-edge biomedical technologies to oral health.


Following many scientific publications that have appeared in journals all over the world, the PerioblastTM team has recently completed the largest clinical study ever conducted on periodontitis. Microbiological tests on nearly 3,000 patients and 14,000 periodontal pockets have fully demonstrated the long-term efficacy of the PerioblastTM protocol. This study was published in the prestigious "European Journal of Clinical Microbiology & Infectious Diseases".

Getting to the root

Periodontitis is a multifactorial, multi-symptom disease: this means that its onset depends on a number of elements, and there is a range of symptoms. In some cases one might suffer from periodontitis but not have evident symptoms. This is why the PerioblastTM protocol seeks to customise the investigation of critical nodes for each case of periodontitis.


It is widely believed that tooth loss is due to advancing age. This belief is only partly true. It is not time that causes teeth to be lost, it is the action of a disease called periodontitis. Periodontitis is a bacterial infection that affects the periodontium, which is the set of tissues that support the teeth. As time goes by the gingival tissue recedes, the bone demineralises, and dental mobility increases. Tooth loss becomes inevitable.

The signs

thanks to the presence of plaque and tartar
(on which they feed), concealing their
action behind signs that are often deemed to
be negligible, common and apparently above suspicion,
such as gingivitis, bleeding, sensitive teeth or halitosis.

Traditional dentistry

Identifying periodontitis using traditional diagnostic techniques or treating it through normal professional oral hygiene is inadequate, and ineffective in the long run. The simple mechanical removal of plaque and tartar does not extend to the periodontal tissues, working only on the surface. This is not enough to eradicate the pathogenic bacteria of periodontitis. In the same way antibiotics, which propagate through circulation in the bloodstream, cannot reach and act on the tissues in the dental zone, which are not highly vascularised.

Clinical studies

The combined efforts of EDN clinics in Italy and IRF in Microdentistry have led to a large number of different clinical studies on periodontal disease and related pathologies, which have been accepted by the scientific community in Italy and internationally (read or download articles here). The PerioblastTM protocol is the result of these intense years of study into the various forms of periodontal disease, and is concrete proof of how scientific research and biomedical technological progress can be applied in daily clinical activity.


In early 2016 the largest study ever conducted on periodontitis was published: a survey on 2,700 patients and 14,000 periodontal pockets that statistically demonstrated the long-term efficacy of the PerioblastTM laser-assisted protocol. Test results showed improvements in all clinical parameters regarding the pathology, even in the most aggressive cases, corroborated by subsequent observations lasting 24 months after the completion of therapy.

Other important results pertain to the sequencing of the microbiome of periodontal pockets and in vitro immune response, comparing pathogenic microbial species with those present in patients’ oral cavities, and using them as diagnostic markers not only for periodontitis but also for major systemic diseases.

Team & Lab

The scientific excellence behind the PerioblastTM protocol comes from the team of dental surgeons and researchers having international experience in this area of study. This team is responsible for numerous publications appearing in the most prestigious journals of the sector and for organising events, conferences, training and refresher courses for all dentistry professionals. The experience of Dr F.S. Martelli and his collaborators led to the project for the creation of a research institute focusing on the study of techniques to treat oral cavity pathologies, an idea that came to fruition in 2001 with the creation of IRF in Microdentistry.

Working in collaboration with other Italian and international research institutes, Istituto per la Ricerca e la Formazione in Odontoiatria Microinvasiva was responsible for drawing up the Perioblast™ protocol. The periodontal risk test is as non-invasive as possible, with cell samples taken by wiping a small sponge on the inside of the cheeks and on the tongue. This test only has to be done once, and serves to calculate one’s personal likelihood of contracting periodontitis. Risk is classified as low, medium, intermediate, high or very high according to the patient’s immune response.


The microbiological test serves to identify the bacterial
and viral population present in the oral cavity during the
various phases of PerioblastTM treatment.

Collected data are first considered in terms of the quantity
of microorganisms present in the mouth, then are broken down
by species class, making it possible to identify the bacterial
or viral groups responsible for the onset of the pathology.

The PerioblastTM microbiological test is crucial for successful
therapy, and its yearly repetition is fundamental for preventing relapses.

Enzyme tests

The enzyme tests forming part of the PerioblastTM protocol are an excellent alarm bell for detecting in advance the presence of a tissue inflammation or damage in the oral cavity thanks to the use of the biomarker aMMP-8 (active matrix-8 metalloproteinase or collagenase).

Enzyme tests serve to detect the risk of degeneration of the gingival, periodontal or peri-implant tissues before traditional diagnostic procedures, such as probing and x-rays, which can only offer evidence of already-present tissue damage. The PerioblastTM protocol thus offers a veritable early diagnosis tool.

The introduction of the laser in dental surgery is a revolution deriving directly from a revelation: access to the infinitely small thanks to the operating microscope. While they are not completely replaceable, traditional surgical tools reveal themselves to be less effective in treating periodontal bacterial infections. The mechanical action of dental ablation, especially if it is carried out under the microscope, is important for eradicating plaque and tartar, a breeding ground for bacteria. Ablation does not however manage to reach the dentinal tubules or jawbone, inside which the bacteria responsible for periodontitis are hidden and continue to act. It is here that the laser gets to work, and thanks to the power of its blue beam it manages to eliminate bacteria in all zones of the periodontium, without having to resort to surgery.

The results

The trial phase of the PerioblastTM protocol therapy demonstrated concrete biological recovery from periodontal disease, a minimal use of antibiotics in the post-operating phase and easy solutions for relapses. But the most impressive result observed is that of maintaining the biological equilibrium and conserving teeth, thus obviating the need for dentures or implants.

Preventing periodontitis certainly depends on careful daily oral hygiene, through the use of flossing, mouth rinses and swab cleaning, in addition to the brushing of teeth (preferably with an electric brush) after meals. It is a good idea to supplement these daily habits with regular professional oral hygiene sessions with the use of the microscope, and taking of the periodontal risk test, which can evaluate the response of the individual immune system in order to predict the possible onset of early or aggressive forms of periodontitis. The periodontal risk test is also useful for predicting the onset of some systemic diseases associated with periodontitis depending on one’s genetic makeup, such as: diabetes, osteoporosis, cardiovascular diseases or infertility.


The success of the PerioblastTM protocol is
not just the biological treatment of periodontitis. Its main goal is clinical recovery, tied up with the programming of a tailor-made maintenance therapy that includes, in addition to the usual oral hygiene practices at home and in the dentist’s surgery, the regular repetition of Perioblast diagnostic tests, in order to prevent the subsequent re-emergence of periodontal problems.
